Patients can pick up registration forms starting Aug. 30 (online version coming soon). 1. Complete the registration form and drop them off at the clinic 2. We will then conduct a phone Meet & Greet starting the week of Aug 30 3. Elderly patients (60+) can come in person for the week of Sept 6 4. After Sept. 7, we will be open to register all other patients.
